Transition Services
Secondary transition is the process of preparing students for life after they leave high school, including participation in post-secondary education or training, employment, and independent living. These three areas are often referred to as “post-secondary outcomes” and are the driving force behind Individualized Education Programs (IEPs) written for ages 14 and above.
Transition Services are a coordinated set of activities for a child with a disability that prepare a child's movement from school to post school. Services are based on individual needs and can include instruction, related services, community experiences, and acquisition of daily living skills.
